What are your financial policies?
I work on a fee for service basis and do not accept insurance. People pay directly for my services and you will be asked to put a credit card on file which will usually be charged on the day of each visit. We can provide a statement of services with diagnostic and service codes which patients with out of network mental health insurance benefits can use to file for themselves. I am not a Medicare or Medicaid provider. Medicare eligible patients must agree to not submit a claim under the Medicare or to Medigap or other supplemental insurance plans. If a prospective patient needs to use a practitioner who accepts their insurance or Medicare, I am happy to look at a provider list and suggest someone whom I think might be a good fit for their needs. Finally, we charge for missed appointments unless you provide notice of at least 24 working hours.
How do I communicate with Dr. Lewis outside of appointments?
Existing patients will have my cell phone number to use for calls or texting regarding scheduling, however, use of that route of communication is undertaken with the understanding that it is not HIPPA compliant.
Brief communications (less than 10 minutes) are at no charge, but more extended communications outside of scheduled office appointments will be charged at the same rate as office visits in 15minute increments.
What if I need a prescription between appointments?
For patients on medication I use an electronic prescription service and transmit patients’ prescriptions to their pharmacy of choice. Ideally prescriptions are managed during office visits. When prescriptions or refills are requested outside of office visits, there will be a charge of $25.
How do I schedule an appointment?
I do my own scheduling and prefer to speak to prospective patients by phone to determine whether I’m a fit for the situation before we schedule an initial consultation.
